PERSONAL ASSISTANT TRAINER & COORDINATOR (Part-Time)
JOB DESCRIPTION
SUMMARY: Administers and promotes the Personal Assistant Referral Program. This includes but not limited to the recruitment and training of Personal Assistants, coordination of Personal Assistant services, and Personal Assistant management training. Meet with seniors with disabling conditions to identify needs and refer and provide them with services to meet these needs.
These services include, but are not limited to, information and referral, individuals and systems advocacy, peer counseling, benefits assistance, assistive technology, and workshops and class offerings. Work collaboratively with staff, and community service providers to bring information about LINC’s Aging & Disability Resource program and services to the whole community.
REPORTS TO: Assistant Executive Director
RESPONSIBILITIES:
Facilitates the Personal Assistant Referral Program. Oversees the maintenance of consumer files for the Home Service Program to ensure proper documentation and maintain up-to-date case notes.
Prepare quarterly reports and evaluate effectiveness and progress in reaching goals and objectives for the fiscal year.
Complete intakes on new consumers referred to the Personal Assistant Referral Program including an assessment of the consumer’s needs and development of an Independent Living Plan.
Interview and review take applications from individuals interested in working as Personal Assistants.
Referral of potential Personal Assistants to consumers within five working days of request or as expediently as possible. Refer qualified Personal Assistants to consumers upon request.
Contact the consumer within two weeks of the consumer hiring a new Personal Assistant to assess the consumer’s satisfaction with the new Personal Assistant and the referral process.
Assist consumers with Personal Assistant management skills as necessary.
Enter and update Personal Assistant information in the computer database. Organize and Maintain Personal Assistant files. Manage and conduct orientation sessions for Personal Assistants, as requested. Assist with mailing for the Personal Assistant Program.
Maintain confidentiality of any and all information that concerns consumers, potential consumers and LINC staff.
Serve as a disability rights advocate to help break down barriers in employment, services and public facilities, etc. Complete all required documentation accurately and in a timely manner. Perform other duties as assigned.
REQUIREMENTS:
Associate degree highly preferred. Excellent Good interpersonal and written communication skills. Experience in working with persons with disabilities preferred. Must have basic computer skills and ability to operate standard office equipment. Must be able to maintain confidentiality of agency and consumer information and maintain a high standard of professionalism.
Experience in working with persons with disabilities preferred. Understanding and demonstrating a continuing commitment to the Philosophy of Independent Living is necessary.
