Job Overview
We are seeking a dedicated and compassionate Case Manager to serve Centre, Clinton and Lycoming Counties, who will be responsible for coordinating and executing various programs, services, and activities for clients. This position is primarily remote with client transportation responsibilities.
Duties
- Evaluates and documents client performance on goal areas and submits reports for personal progression such as progress notes and monthly summaries.
- Maintain work schedule to ensure clients’ needs are met.
- Provides independent living skills training and support in the person's home and community.
- Provides social services to assigned caseloads that assist clients to attain or maintain the highest level of independence.
- Gathers information and provides guidance to potential clients seeking services, while connecting and facilitating appropriate local resources for assistance. Compiles, maintains, and audits client records for services received and maintain client confidentiality.
- Identifies ways to accommodate clients’ choices, preferences, and customary routines. Includes these approaches in service plan and provides the information to the client.
- Assists Case Manager Supervisor with yearly contract audit, monthly records of services and annual client file reviews, month end and contract billing, and social recreational services.
- Promote good eye health and safety to the community through educational programs and maintain inventory and distribution of educational materials.
- Provide transportation for clients to necessary appointments utilizing company resources and schedule routine vehicle maintenance.
- Facilitate new referrals from initial phone calls, first visit, referrals to internal services (AT, ISI), plus referrals to other service providers
- Coordinate group activities for clients that promote quality of life
- Work in a state of alertness, and adhere to a safe, secure, and healthy work environment.
- Other duties as assigned by the Case Manager Supervisor.
- This is a travel position, with target area of Centre, Clinton and Lycoming Counties. Occasional presence at the home office in Williamsport will be required.
- A company car will be provided.
- SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities. __________________________________________________________________________________
QUALIFICATIONS:
- Must be able to follow instructions and keep accurate records.
- Possess empathy and understanding for the needs of people who are blind or visually impaired.
- Knowledge of personal computer systems is required.
- Must be able to perform each essential duty satisfactorily.
- Must be able to work independently.
- Must have a valid PA driver’s license
- EDUCATION & EXPERIENCE:
- Coursework in the area of special education or social work preferred, though not required.
- Associate's or experience preferred but not required.
- SKILLS & ABILITIES:
- Strong skills in personal computer operation with understanding of Microsoft Office Suite.
- Demonstrated ability to actively participate and perform in a team-oriented environment.
- PHYSICAL REQUIREMENTS*: The physical demands described are representative of those that must be met by an associate to successfully perform the essential functions of this job including: Ability to sit, stand, and lift a minimum of 25 pounds independently, and more with assistance. Specific vision abilities required by this job include close vision, and ability to adjust focus.

Pay: $18.00 - $19.50 per hour
