Position Overview
Legacy Concierge is seeking dedicated and compassionate Private Duty Registered Nurses (RNs) to deliver exceptional 1:1 care to patients in the comfort of their homes. This role is designed specifically for RNs who prefer direct patient care — you will not be responsible for opening admissions or opening assessments.
Your focus will be delivering hands-on clinical care and fostering continuity and trust with your assigned patient during 12-hour shifts.
Schedule
- 12-hour shifts (Day or Night; schedules may vary depending on patient needs)
- Part-Time, and Per Diem positions available
Key Responsibilities
Patient-Focused Clinical Care
- Deliver 12-hour, 1:1 skilled nursing care in the patient’s home.
- Perform clinical tasks based on the patient’s plan of care, which may include:
- Medication administration and management
- Tracheostomy and ventilator care (if required)
- G/J-tube feeding and suctioning (if required)
- Wound care and dressing changes
- Seizure monitoring, oxygen therapy, or other chronic condition management
- Monitor and document vital signs and patient status throughout your shift.
- Provide emotional and physical support to the patient and their family.
Non-Administrative Focus
- No Start of Care, Recertification, or Discharge assessments.
- No case management or patient intake responsibilities.
- Communicate changes in patient condition to clinical supervisors and follow established care plans.
Documentation & Reporting
- Complete daily clinical notes using the agency’s EMR system.
- Report incidents or clinical changes promptly to the supervisory nurse.
- Maintain compliance with HIPAA, infection control standards, and internal protocols.
Schedule & Compensation
- 12-hour shifts available (days and/or nights)

Experience:
- Registered Nurse: 3 years (Required)
