A leading Skilled Nursing and Rehabilitation Center in Columbia County, Florida is seeking a dedicated and compassionate Social Services Director to join our leadership team. In this vital role, you will ensure residents and their families receive the emotional, social, and mental health support they need while maintaining compliance with all state and federal regulations.
As a key member of our interdisciplinary team, you will oversee social service programs, lead discharge planning efforts, advocate for resident rights, and foster strong community partnerships to enhance resident well-being and quality of life.
Direct and coordinate resident discharge planning, financial assistance programs, and community referrals.
Ensure residents receive behavioral health services aligned with individualized care plans.
Facilitate resident and family council meetings and provide education on resident rights, advance directives, and available resources.
Maintain compliance with state and federal regulations and assist with policy development and implementation.
Provide culturally sensitive support and translation assistance when needed.
Maintain social services documentation and resource materials in accordance with regulatory requirements.
Collaborate with nursing, therapy, dietary, activities, and administration teams to improve resident outcomes.
Participate in quality assurance initiatives, surveys, inspections, and regulatory reviews.
Develop and maintain relationships with community agencies and healthcare partners to support resident needs.
Advocate for residents and families while promoting a positive and supportive environment.
Bachelor's Degree in Social Work or a related field required.
Minimum of two (2) years of supervised social work experience in a skilled nursing facility or healthcare setting.
Strong understanding of Medicare, Medicaid, and skilled nursing care processes.
Knowledge of state and federal regulations governing long-term care facilities.
Excellent leadership, communication, and organizational skills.
Ability to work effectively with residents, families, interdisciplinary teams, and community agencies.
