Job Title: Hospice RN Float – Admissions, Case Management & Field Visits (RN)
Pay: $90,000 – $130,000 DOE
Benefits: Full Benefits Package
Schedule: Monday – Friday with on-call and weekend rotation
Location: Greater Twin Cities Metro Area, 21-County Service Area

What You’ll Do
Admissions: Complete evaluations, eligibility assessments, education, consents, and care planning.
Case Management (as needed): Manage a caseload including symptom management and care coordination.
Field Visits and Crisis Support: Provide routine, PRN, and urgent visits while collaborating with providers.
Care Coordination: Partner with social workers, aides, and chaplains to support patient goals.
Documentation and Compliance: Maintain accurate EMR documentation and meet hospice regulations.
What You’ll Bring
Active Minnesota Registered Nurse license.
Hospice experience preferred or strong background in ICU, oncology, or palliative care.
Strong admissions skills, clinical judgment, and family communication ability.
Highly organized and comfortable working in a fast-paced, field-based role.
Reliable transportation and ability to travel throughout the service area.
A compassionate, patient-centered mindset with a passion for hospice care.
