Job Title: LPN Home Health
Assignment Length: 13 weeks with possible extension
Location: Northampton, MA
Shift: Monday – Friday (8am -4:30 pm) with weekend rotation of 1 /4 weekend requirement.
Pay Rate: $44/hr
Job Summary:
LPN position provides 6 patient visits per day and works within teams of RNs, PT, OTs, HHAs. This position requires home care experience. Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, responsible for performing skilled nursing functions in accordance with the State Nurse Practice Act and acceptable standards of practice, federal, state, and local laws and within the guidelines of his/her professional license.
The LPN delivers care to patients as delegated by the RN Case Manager and/or Clinical Team Manager. This position is required to work one weekend shift per month.
Experience:
- At least 1 year of LPN experience.
- At least 1 year Home Care/home health experience.
Job Requirements:
- Active Massachusetts LPN License
- Current BLS Certification from AHA
- Fully Vaccinated against Covid-19
