Job Title: Registered Nurse – Program Coordinator for Portable Clinical Care

NJCRI is seeking an experienced and self-reliant Registered Nurse (RN) with a background in addiction
nursing to join our team as a Program Coordinator for our Portable Care Program. The ‘Street Medicine’ RN is responsible for the clinical oversight, coordination, and delivery of mobile and community-based healthcare services for individuals experiencing homelessness and other underserved populations. This role combines direct patient care, program management, community outreach, staff supervision, and partnership development to ensure high-quality, patient-centered care. The Program Coordinator oversees nursing operations, ensures compliance with clinical standards, and supports strategic initiatives to expand access to healthcare services in community settings.
Clinical Services and Direct Patient Care
· Provide nursing care in community, shelter, encampment, and mobile health settings.
· Coordinate and participate in street outreach, mobile health services, and community engagement activities.
· Conduct HIV, viral hepatitis, and sexually transmitted infection (STI) testing, counseling, education, and linkage to treatment services.
· Perform phlebotomy and specimen collection for laboratory testing.
· Conduct comprehensive health screenings, including blood pressure, blood glucose, cholesterol, pregnancy testing, and other point-of-care assessments.
· Review laboratory results and coordinate follow-up care with patients and healthcare providers.
· Provide wound assessment, treatment, and ongoing wound care management.
· Administer vaccinations and other preventive health services as permitted by scope of practice.
· Deliver overdose prevention education and facilitate access to Naloxone.
· Assess patients for acute and chronic medical conditions and coordinate appropriate interventions.
· Facilitate referrals to primary care providers, infectious disease specialists, behavioral health services, substance use treatment programs, social services, housing resources, and other supportive services.
· Coordinate pregnancy testing and referrals for prenatal and reproductive healthcare services.
· Monitor client progress and ensure successful linkage and retention in healthcare services.
Program Management and Operations
· Manage daily operations of the Street Medicine Program, including clinical workflows, outreach activities, and service delivery.
· Develop, implement, and evaluate program policies, procedures, and quality improvement initiatives.
· Ensure compliance with federal, state, local, and organizational regulations, policies, and standards.
· Monitor program performance metrics and outcomes to ensure achievement of grant and agency objectives.
· Generate and submit monthly, quarterly, and annual reports as required by funding agencies and organizational leadership.
· Routinely collect client feedback and patient satisfaction surveys to inform program improvements.
· Maintain accurate documentation, case notes, and data entry within electronic health records and reporting systems.
Health Education and Special Projects
· Develop and deliver health education programming through in-person outreach, community events, and virtual platforms.
· Support online health education and outreach initiatives utilizing organizational social media and digital engagement platforms.
· Participate in special projects, pilot programs, research initiatives, and other duties as assigned.
Job Qualifications & Skills:

- Licensed as a professional registered nurse in New Jersey.
- Demonstrated ability to work with people from diverse backgrounds and interests.
- Phlebotomy
- Knowledge of infectious diseases, prevention, and risk reduction strategies as well as familiarity with the social service delivery system particularly in regard to substance abuse and mental health.
- Critical thinking, decision-making skills, proficiency in written and verbal communications.
- Bilingual (Spanish/English desirable).
- Microsoft Office (Outlook, Excel, Power Point and Word)
- Valid NJ driver's license.
Pay: $85,000.00 - $90,000.00 per year
