Job Summary
We are a multi-specialty primary care practice seeking a highly skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) with a focus on substance abuse and addiction treatment to join our integrated healthcare team. In this role, you will lead behavioral health and substance use services within a collaborative, whole-person care model, partnering closely with our primary care providers and specialists to treat the full range of adult patients we serve. Your primary responsibilities will include:
Duties
- Conducting psychiatric evaluations, mental status exams, and comprehensive biopsychosocial and substance use assessments
- Overseeing the Withdrawal Management (detox) Program
- Overseeing the Medication-Assisted Treatment (MAT) Program, including Suboxone (buprenorphine/naloxone) induction, titration, and maintenance
- Prescribing and managing Suboxone and other MAT medications (buprenorphine, naltrexone/Vivitrol) for opioid use disorder
- Diagnosing and treating substance use disorders alongside co-occurring psychiatric conditions, including mood, anxiety, psychotic, and trauma-related disorders
- Prescribing and managing psychotropic medications
- Providing integrated behavioral health and addiction support for patients with co-occurring medical and psychiatric conditions referred by our primary care and specialty providers
- Ordering and interpreting diagnostic and laboratory tests, including urine drug screens and medication-level and metabolic monitoring
- Developing and implementing individualized treatment and recovery plans coordinated with the patient's primary care team
- Providing supportive counseling, psychoeducation, and relapse-prevention guidance
- Conducting risk assessments for suicide, self-harm, overdose, and safety concerns
- Collaborating with primary care physicians, specialists, therapists, case managers, and other healthcare professionals to ensure coordinated, patient-centered care
- Maintaining accurate and up-to-date medical and psychiatric records in compliance with regulatory standards
