At Home Instead, we believe aging should be met with dignity, independence, and compassionate support. We're looking for a caring, organized, and relationship driven Registered Nurse (RN) to join our team as a Client Coordinator .
What You'll Do:
- Conduct in home care consultations and client assessments.
- Develop individualized plans of care that promote safety, independence, and quality of life.
- Coordinate services and collaborate with clients, families, Care Professionals, and community partners.
- Train and support staff direct care workers.
- Complete quality assurance visits to ensure exceptional care and client satisfaction.
- Serve as a compassionate advocate and trusted advisor for clients and their families.
What We're Looking For :
- Active West Virginia Registered Nurse (RN) license.
- Previous experience in home care, home health, hospice is preferred.
- Experience working with older adults.
- Strong organizational skills.
- Excellent communication, interpersonal, and organizational skills.
- Ability to travel throughout our service area for client visits.
- Flexibility to work occasional evenings and weekends as needed.
