We provide personalized, one-on-one physical therapy in patients’ homes, helping individuals improve mobility, strength, balance, safety, and functional independence.
A Different Approach to Outpatient Physical Therapy
This is not a traditional clinic-based position.
At TheraMove, therapists work one-on-one with patients without double-booking or the demands of a high-volume clinic environment. Our mobile outpatient model gives clinicians the opportunity to provide individualized, unrushed care while maintaining flexibility and professional autonomy.
We are seeking a full-time licensed Physical Therapist who is confident working independently, values high-quality patient care, and enjoys developing meaningful relationships with patients and their families.
Responsibilities
- Perform comprehensive physical therapy evaluations in patients' homes
- Develop individualized plans of care based on each patient's functional limitations, goals, and clinical presentation
- Provide skilled interventions including therapeutic exercise, therapeutic activities, neuromuscular re-education, manual therapy, gait training, balance training, and functional mobility training
- Address mobility, strength, balance, endurance, pain, fall risk, and functional independence
- Provide patient and caregiver education to promote safety, independence, and long-term management
- Monitor patient progress and modify treatment plans based on clinical response and functional outcomes
- Complete accurate and timely documentation using our electronic medical record system
- Communicate professionally with patients, caregivers, physicians, and other members of the healthcare team
- Travel to patients within assigned Colorado Springs service areas
Qualifications
- Active Colorado Physical Therapist license
- Strong clinical reasoning and ability to independently develop and progress individualized plans of care
- Outpatient, orthopedic, geriatric, or home-based therapy experience preferred
- Comfortable working independently in a mobile outpatient environment
- Strong documentation and time-management skills
- Excellent patient education and communication skills
- Ability to establish rapport and maintain professional relationships with patients and caregivers
- Organized, dependable, and self-motivated
- Reliable transportation and willingness to travel locally between patient visits
- Experience with HelloNote or similar EMR systems is a plus
