Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary
- Monitor patient health by observing vital signs, managing medication administration, and reporting changes promptly using Electronic Health Record (EHR) systems or Medical Records software
- Support clients with Alzheimer’s care, dementia care, and memory care needs by implementing specialized techniques to ensure safety and comfort
- Perform medical tasks including catheter care, wound care, and assisting with hospice or palliative needs in accordance with medical terminology and protocols
- Prepare nutritious meals and assist with meal planning, cooking, laundry, cleaning, and maintaining a safe home environment
- Document all patient interactions accurately while adhering to HIPAA regulations to protect patient privacy and confidentiality
- Utilize medical documentation skills to record patient progress and communicate effectively with healthcare teams for coordinated care
Qualifications
- Prior experience in assisted living facilities, nursing homes, or home health environments is highly preferred
- Strong knowledge of medical terminology, vital signs monitoring, medication administration, and dementia or Alzheimer’s care techniques
- Experience working with individuals with developmental disabilities or disabilities requiring long-term care support
- Certification in CPR is required; additional certifications such as hospice care training or working with EHR/EHR systems are advantageous
- Ability to lift patients safely using Hoyer lifts or other assistive devices; physical stamina for long shifts is essential
- Excellent patient observation skills combined with compassionate caregiving abilities
- Familiarity with HIPAA regulations and maintaining strict confidentiality in all patient interactions
