Schedule: Monday - Friday 8 AM-5 PM
Service Area: Greater Milwaukee area.
Requirements
Must have at least 1 year of Home Health experience within the last 3 years as an RN.
HCHB and Oasis charting experience is required.
Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and evaluation of physiological status to develop personalized care plans.
- Manage patient care through comprehensive clinical case management, discharge planning, and coordination of services across home health settings.
- Utilize Electronic Health Records (EHR) systems to document medical records accurately, ensuring compliance with HIPAA regulations and health record policies.
- Perform diagnostic evaluations and coordinate with healthcare professionals to optimize patient outcomes in post-acute and outpatient environments.
- Provide patient education on medication management, disease processes, and health promotion strategies tailored to individual needs.
- Monitor patients’ progress through regular visits, adjusting care plans as necessary based on clinical findings and patient response.
- Collaborate effectively with physicians, therapists, social workers, and other healthcare providers to ensure seamless continuity of care and optimal patient service.
Experience
- Proven experience in home health nursing or post-acute care environments with a strong foundation in medical-surgical, inpatient, or outpatient settings.
- Critical care experience including ICU or Emergency Room backgrounds is highly desirable for managing complex cases efficiently.
- Familiarity with Level I Trauma Center protocols, infectious disease care, behavioral health considerations, and geriatrics enhances your ability to handle diverse patient populations.
- Demonstrated knowledge of hospital medicine practices, discharge planning procedures, and utilization management processes.
- Experience working with Electronic Health Records (EHR) systems such as EMR platforms; proficiency in medical terminology is essential.
- Strong understanding of health regulation policies including HIPAA compliance; excellent patient service skills and client communication are vital.
- Ability to perform detailed patient assessments, manage multiple cases simultaneously as a nurse case manager, and coordinate diagnostic evaluations effectively.
