American Traveler is seeking an experienced Home Health RN with a minimum of 2 years of experience and OASIS documentation proficiency for a 13-week travel contract in Vermont.

Home health setting serving a patient population with complex care needs including wound care, IV therapy, and symptom management
-
Serves as Case Manager for assigned patients, managing caseloads and visit times in accordance with agency productivity standards
-
High-tech skills required including IV use and maintenance, wound VAC, and complex wound care
-
Uses McKesson for charting and documentation

OASIS documentation is a core part of this role
-
Schedule is Monday–Friday, 8am–4pm, with some on-call and weekend requirements

Active RN license in Vermont (VT) or a compact RN license that is currently valid and in hand

Minimum 2 years of RN experience required

OASIS documentation experience required

Proficiency with McKesson EMR

Skills in IV therapy, wound VAC, and complex wound care required

Conducts comprehensive patient and caregiver assessments using the nursing process to develop and implement individualized plans of care

Collaborates with interdisciplinary team (IDT) members including physicians, LPNs, physical therapists, social workers, and other providers to coordinate patient care

Participates in IDT meetings, case conferences, and monthly peer review audits

Performs patient and caregiver education on topics including medication side effects, treatment options, discharge instructions, and reportable conditions

Responsible for accurate and timely completion of agency, state, and federal forms including HHABN and Medicare Notice of Non-Coverage

Identifies and communicates interdisciplinary patient needs in a timely manner to ensure continuity of care

Coordinates safe patient discharges and/or transfers, ensuring all required documentation is completed per policy
