At Catalytic Health Partners (CHP) our Social Needs Coordinator will ensure members’ social needs are met by directly coordinating with providers and external community groups to assist in closing gaps in members’ social support needs. This coordinator oversees all requests of members, including those assigned to the providers, to ensure there are no gaps in social support provided to the members.
What We'll Expect from You and What You'll be Doing:
- Establish working relationships and trust with members by phone to learn the details of their needs and how their needs can be met.
- Will oversee and be accountable to the requests of your members, which include those of the field staff. Fulfill various requests to meet the members’ social needs in an appropriate time outlined by the team and leadership.
- Research resources to meet the members’ needs. Locate resources that are scarce and often hidden under multiple layers of organizations, learning how the application and approval process works, and helping members to succeed in applying for these resources.
- Build relationships with community partners to understand their respective organizations, services and resources offered. Learn about these organizations’ processes for application and approval and what their needs are so CHP can identify how they can more strongly partner with them for mutual success.
- Complete an audit of members’ AHCCCS insurance twice a month to ensure that they are still insured, and all proactive steps can be taken to keep the member on AHCCCS.
- Meet the members' needs by assisting them with government applications that include food stamps, phones, medical insurance, etc.
- Assist the member by finding housing resources and helping them apply. Will see members through this process from start to finish in order to ensure the member’s needs are met.
- Help in finding new resources in the community and update the resource database so all team members can see and access information about the new resources.
- Coordinate all medical transportation for members to ensure that the member is safely picked up and dropped back off. Update the team on what is learned from the members about their visits.
- Will update and coordinate with two provider groups to help them meet member’s needs and to coordinate the appropriate care. Each provider group consists of a Nurse Practitioner, a Behavior Health Therapist, a Care Coordinator, and a Clinical Liaison.
Shift: 10am-7pm
Required Qualifications:
- Legally authorized to work in the US.
- High school diploma or equivalent.
- Experience in a customer service environment. Excellent communication skills, both in explaining complex information and in actively listening to patient needs.
- Ability to multitask among many tasks throughout your day.
Preferred Qualifications:
- Strong skills in organization, time management, and adaptability to change.
- Empathy and teamwork skills are highly valued.
- Ability to prioritize tasks to ensure efficient completion of all needs.
- Working ability in Microsoft products especially Word, Excel, Outlook and OneDrive.
- Ability to navigate computer systems while also being on phone.
