Job Location: Lewisburg, WV and surrounding areas
Position Summary:
Provides skilled nursing care, case management, and other clinical support services to patient/families in the home setting.
Performs other related duties as required.
Position Qualifications:
Graduate of an approved nursing program.
Current unrestricted WV RN license.
Strong organizational, problem-solving, and people skills.
Contingent upon successful completion of drug screen, background check, and other screenings as applicable.
Work Environment:
Exposure to infectious diseases, bloodborne pathogens, and bodily fluids.
Frequent interaction with patients, families, and interdisciplinary team members.
Fast-paced clinical settings with potential for high stress.
Required use of personal protective equipment (PPE).
Physical Requirements:
Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance.
Frequent standing and walking for extended periods (8–12 hour per shift).
Frequent bending, stooping, kneeling, crouching, and reaching.
Ability to push and pull equipment (e.g., wheelchairs, stretchers, and medication carts).
Visual acuity to read medication labels, physician orders, and monitors.
Auditory ability to assess patient needs and respond to alarms or calls for assistance.
