Job Summary
We are seeking a dedicated and compassionate Trabajador Social Clínico to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to individuals and families, supporting their mental health, emotional well-being, and overall quality of life. Your expertise will help facilitate access to resources, deliver therapeutic interventions, and coordinate care plans tailored to each client’s unique needs. This position offers an opportunity to make a meaningful impact in diverse clinical settings while working collaboratively with multidisciplinary teams. The ideal candidate is energetic, motivated, and committed to advancing patient-centered care through evidence-based practices. The population is adults form age range of 18 to 80 institutional service facility in Guayama PR. The service is two to four visits per month.
Responsibilities
- Psychosocial assessments
- Intervention and case management
- Coordination of community and family services
- Guidance and support for families
- Development of discharge or transition plans when applicable
- Coordination with external agencies and resources
- Monitoring of compliance and social needs
- Required documentation and clinical records
- Intervention in psychosocial crisis situations
Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited program; licensure as a Licensed Clinical Social Worker (LCSW) or equivalent preferred.
- Proven experience in clinical social work within hospital settings or community-based programs.

Pay: $700.00 - $5,000.00 per month
