The licensed practical nurse (LPN) provides nursing care to assigned clients under the RN Case Manager, or other assigned supervisor, and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. This position collects client data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN delivers in-home care in accordance with the client plan of care established by the RN Case Manager. The LPN must possess excellent communication skills and approach tasks with a positive and flexible attitude. Participation in team meetings, case conferences and in-service education is required.
QUALIFICATIONS:
Minimum of one year LPN experience (home health care experience required)
Background check required
LICENSE OR CERTIFICATION:
Current Colorado Licensed Practical Nursing License
Current CPR Training Card
Current Driver’s License
Job Types: Full-time, PRN
Pay: $40.00 - $45.00 per hour
