The Case Management Registered Nurse - Podiatry/Float plays an important role in helping Veterans, families, and caregivers access the right care at the right time. This position supports Veterans with complex medical, psychosocial, and care coordination needs while working closely with providers and interdisciplinary team members across care settings. Through thoughtful planning, patient education, advocacy, and outcome evaluation, the RN helps strengthen continuity of care, improve the Veteran experience, and contribute to the mission of delivering high-quality, Veteran-centered health care.
Duties include, but are not limited to:
- Coordinate care for Veterans across inpatient and outpatient settings to support timely access to appropriate services and resources.
- Assess, plan, implement, and evaluate nursing care using age-specific and Veteran-centered approaches.
- Support discharge and disposition planning to promote safe transitions and continuity throughout the care continuum.
- Assist Veterans, families, and caregivers in understanding care options, services, and self-management strategies for complex health needs.
- Collaborate with interdisciplinary team members to address medical, nursing, and psychosocial needs that affect care outcomes.
- Participate in interdisciplinary patient care management activities to align care plans with Veteran needs, preferences, and goals.
- Evaluate care delivery and outcomes to help ensure services are timely, appropriate, coordinated, and responsive to patient needs.
- Apply evidence-based practices and participate in unit-level improvement activities that enhance quality, efficiency, and patient-centered care.
