JOB TITLE: Physical Therapist Assistant
MAIN FUNCTION: Administers physical therapy to residents.
REPORTS TO: Registered Physical Therapist, Director of Nursing Services and/or Executive Director in absence of RPT
DUTIES AND RESPONSIBILITIES:

Administers physical therapy to patients while under the supervision of and as assistant to the physical therapist.

Instructs, motivates and assists patients in performance of exercise programs, transfer and ambulation techniques.

Observes patient responses to therapy and reports orally or in writing to the physical therapist

Reports patient responses to therapy and/or status to Home staff when necessary.

Writes progress notes on patients designated by the RPT. Notes to be co-signed by registered therapist.

Complete team conference forms and MDS+ as needed. Attends team conference and serves as chairperson when indicated.

Provides in-service as requested.

Compiles monthly statistical data on patients visits and treatments. Is responsible for obtaining physician orders on evaluations, filing of evaluations and record keeping of patient status.

In the absence of the physical therapist supervises all operations of the department. Is responsible for supervision and direction of the physical therapy aides.

Performs other duties as requested.
CONTACTS: Frequently with residents, employees in own and other departments, visitors and families. Sometimes house staff, supervisors and department heads. Occasionally with physicians.
PHYSICAL DEMANDS: Stands, walks most of the day. Works in reaching or strained positions most of the day. Pushes, pulls, moves and lifts residents most of the day.
WORKING CONDITIONS: Daily exposure to soiled linen and occasionally body wasted.
HAZARDS: Possible injury to self (back strain).
SAFETY: Will follow safe work practices in accordance with facility health and safety policies.
EDUCATION AND EXPERIENCE: Must have successfully completed the New York State licensing exam. A minimum of two years experience.
