Star Choice Health is looking for skilled Physicians to work in sub-acute and long term care settings.
Position Highlights:
- Create partnerships with facilities and provide effective communication at all times.
- Collaborate with other healthcare professionals to develop and implement effective patient care plans.
- Diagnose and treat acute and chronic illnesses, injuries, and conditions.
- Prescribe medications and other treatments, including psychopharmacology.
- Perform evaluations on patients with changes in condition upon facilities request .
- Order and interpret diagnostic tests, such as laboratory and imaging studies.
- Adhere to infection control protocols to ensure patient safety.
- Document promptly all patient encounters and maintain accurate medical records using electronic health record systems.
- Attend to patients and their loved ones, providing information and education about preventive care, health promotion, and disease management as needed.
-Coordinate schedules and patient needs through our Administrative office.

Qualifications
- Medical degree with a focus on Primary Care Medicine or related field
- Strong communication skills for effective Patient, Family and Facility interaction
- Experience with care plans and patient management preferred
- Proficiency in EHR systems
- BC/BE
