Duties
- Conduct thorough physical examinations and patient assessments, including vital signs, medical history reviews, and physiological evaluations.
- Develop and implement individualized care plans for geriatric patients
- Manage complex cases palliative medicine
- Utilize electronic health record (EHR) systems like ChartPath and PointClickCare patient encounters for accurately while ensuring compliance with HIPAA regulations.
Qualifications
- 2 years Nurse Practitioner experience REQUIRED.
- Valid Nurse Practitioner license with current certification in advanced practice nursing.
- Demonstrated experience working with diverse populations including children, seniors, individuals with disabilities or developmental disabilities, and those in assisted living or nursing home environments.
- Proficiency in EMR/EHR systems such as Epic or Athenahealth; experience with Cerner or similar platforms is a plus.
- Strong knowledge of medical terminology, physiology, ICD coding (including ICD-9/10), CPT coding, and utilization review processes.
- Skills in pain management—including acute pain management—and behavioral health assessments are preferred.
- Excellent communication skills for patient education and case management; familiarity with occupational health protocols is advantageous.
Pay: $115,000.00 - $125,000.00 per year
