General Description:
This person is responsible for directing the process of interviewing clients to assess their needs and eligibility for services at the Marietta Client Services Center and maintaining detailed client data. In addition, this person conducts intakes, assessments, determines eligibility, links individuals/families with community-based services and resources, manages financial assistance funds and provides case management services in conjunction with community partners.
Essential Responsibilities:
- Represent MUST Ministries in the most positive manner with clients, suppliers and the community we serve. Interact effectively with a diverse group of staff, volunteers, and clients of our service, learn and use operating practices of MUST Ministries.
- Uphold the Mission Statement: Serving our neighbors in need … transforming lives and communities in response to Christ's call.
- Handle all information with tact and discretion and recognize the confidential nature of MUST Ministries.
- Work with Client Intake Coordinator to establish the process to conduct initial client interviews to assess needs and eligibility.
- Screens all applicants and determines the eligibility of applicants; makes appropriate referrals as needed.
- Provides Case management for individual clients including conducting needs assessments, creating individualized service plans to achieve self-sufficiency. Increase client’s awareness of available community and agency resources and assist them in accessing these services.
- Coordinates the referral for substance abuse, employment, and mental health services with the appropriate referral agency/program.
- Serves as the program representative at team meetings presenting the issues and concerns of the clients.
- Work with Client Intake Coordinator to ensure client traffic flow through program areas is efficient and effective.
- Review program policies and procedures with all clients.
- Prepare monthly reports and program statistics.
- Perform other duties as required.
Essential Requirements:
- B.S., Human Services, or related field required. 2+ years of experience working with mental health, disabled, veterans and/or diverse populations. Highly effective in working objectively with a diverse group of people.
- Demonstrates cultural competence and compassion in dealing with clients, staff and volunteers.
- Must demonstrate excellent communication, organizational, and administrative skills.
- Proficiency in Microsoft applications including Word, Excel and Outlook, Google OneDrive and data systems; Salesforce preferred.
- Excellent customer service skills
- Physical and Work Requirements:
- Requires standing, walking, lifting, and bending.
- Work is performed in a day service environment.
- May be required to lift up to 25 lbs.
