Job Summary
We are seeking compassionate and dedicated Community-Based Rehabilitation Services (CBRS) providers to support individuals with mental health challenges, developmental disabilities, and other special needs within their homes and communities. This role involves delivering personalized care, promoting independence, and enhancing quality of life through direct support and community engagement. The ideal candidate will possess strong communication skills, a genuine desire to assist others, and experience working with diverse populations requiring behavioral health services.
Qualifications:
- To work within the scope of a certified CBRS worker, Licensed Social Worker(LSW), or bachelor level paraprofessional.
- Must possess a minimum of a bachelor’s degree.(preferably in a Human Services domain, but not required) Must be licensed if a social worker.
- Complete and successfully pass an Idaho Health and Welfare enhanced background check at https://chu.dhw.idaho.gov
- CBRS worker must have reliable transportation, a current valid driver’s license to operate a vehicle in Idaho, and current full, comprehensive insurance coverage on the vehicle.
- Previous experience as a PSR/CBRS worker is preferred, but not required.
- Paraprofessional must possess and maintain the required certification and education requirements necessary to fulfill this role.
Responsibilities
- Provide direct support to individuals with behavioral and mental health challenges, developmental disabilities, and other disabilities in home and community settings
- The CBRS worker will provide skills building services in the following settings; in the community, in client homes, in the agency office, in schools, or in other established and approved facilities.
- CBRS services will be provided primarily in person, but occasionally via telehealth as approved by agency and regulatory authorities on a client needs basis.
- The CBRS worker will gather through direct client contact, observations, and collaboration with collateral contacts the information pertinent to completing their duties in this role according to state, agency, and other regulatory requirements, including Idaho Medicaid.
- CBRS workers have the option to become certified to perform the CANS assessment and provide these assessments for a child or adolescent client as part of the collaborative treatment team.
- CBRS worker will develop and write treatment plans in coordination with the client, client’s family, and other collateral contacts.
- The CBRS worker will assist in facilitating the coordination of care between a client’s primary care physician, parents, teachers, treatment team members, and any other collateral contacts pertinent to the client’s behavioral health. The CBRS worker will document these efforts in the client’s chart as required.
- The CBRS worker will review the client’s clinical assessments, such as the comprehensive diagnostic assessments, and also collaborate and follow instructions of the providing clinician.
- CBRS worker will complete accurate daily log notes according to agency guidelines and templates. These notes will document the service that was provided, interventions used, and goals addressed, among other details of information required by the agency and other regulatory entities.
- CBRS worker will participate in staff meetings, trainings, and supervision meetings as required by regulatory entities and the agency.
- CBRS worker will provide interventions which are pertinent and evidence-based in nature.
- CBRS worker will provide reports, notes, and other documentation as required by the agency and state protocols. This includes assessments, progress notes, treatment plans, treatment plan reviews, billing sheets, and discharge summaries. This documentation will be completed in a timely fashion per the time restraints required.
- CBRS worker will utilize Electronic Health Records notation and templates created and selected by the agency or other regulatory entity.
- CBRS worker will abide by the ethical codes and standards of their governing and licensing boards or certifications.(i.e. PRA code of ethics) The CBRS worker will make every effort to make sound ethical decisions and utilize supervision and consultation within the agency system to assist in this task.
- The CBRS worker will obtain annual continuing education credits as required by regulatory boards in order to maintain licensure, authority to practice, or certification. The CBRS worker will provide copies of these hours and any license renewals to the agency to be placed in their personnel file.
- The CBRS worker may participate in on-call crisis phone rotations if the worker is trained to provide crisis interventions.
- The CBRS worker will become familiar with, and follow, agency policies and procedures.
- The CBRS worker will maintain confidentiality with respect to HIPAA and other privacy laws pertinent to the nature of the services provided.
- The CBRS worker will work towards PRA certification or other required certification if not currently certified. The CBRS worker may work in the role of a CBRS worker for up to 30 months while working towards the certification. In the meantime, the CBRS worker is expected to use professionalism, appropriate rendering of services, and ethical practices pertinent to PRA and agency standards.
- The CBRS worker will participate in Child and Family Team(CFT) meetings as required.
- Implement behavior management strategies to promote positive behaviors and social skills
- Support individuals in participating in community activities, outings, and social events
- Ensure safety protocols are followed at all times during service delivery
