Responsibilities:
Develop and maintain relationships with residents, family members, and caregivers to understand their needs and preferences
Identify and address any issues or concerns related to our activity program or residents' care
Participate in interdisciplinary team meetings to share information and collaborate on resident care
Stay up-to-date with industry trends and best practices in activity programming and social work
Complete monthly activity calendars
Help assist resident and family with a safe discharge
Represent Shelby Skilled Nursing and Rehabilitation at community events and conferences
Requirements:
