Duties
- Conduct comprehensive hospice patient assessments and develop individualized, cost-effective care plans in collaboration with healthcare providers.
- Coordinate care transitions and work closely with physicians, social workers, discharge planners, and other professionals to ensure seamless patient care.
- Arrange non-medical support services, such as housing and transportation, to improve treatment adherence and patient outcomes.
- Monitor clinical, functional, and financial outcomes, maintain accurate documentation, and promote health while preventing complications.
- Advocate for patients by protecting privacy and confidentiality while applying strong clinical judgment and hospice nursing expertise.
