Continuum of Care Doorways Program Case Manager
Reports To: Executive Director of Randy Sams Outreach Shelter is the immediate supervisor, and the City of Texarkana, Texas is an indirect supervisory role as they are the overseer of this grant which they applied for as the lead applicant to the Texas Homeless Network.
General Description: The Case Manager will devote 100% of time to the Doorways Rapid Re-Housing Program and related activities. The role of the Doorways Case Manager is to provide persons with quick access to housing, services, other service agencies and community resources to help ensure that clients receive all necessary services and subsequently attain self-sufficiency status. The Doorways Case Manager will address individual needs for transportation, employment, medical/mental health issues and assist residents through the application process for available resources while guiding them through the process of obtaining housing of their choosing. Additionally, he or she will work cooperatively with the Executive Director, the Shelter staff, the City of Texarkana, Texas, Mission Texarkana, as well as with other community agencies to find additional resources for their clients, and serve as an advocate against systematic barriers to homelessness. The Doorways Case Manager works collaboratively within the organizational team to provide the most appropriate services available. Randy Sams Outreach Shelter is an at-will employer.
Specific Responsibilities:
- Provide an initial intake and assessment of potential participants utilizing VI-SPDAT, Coordinated Entry procedure and HMIS
- Enter case management notes for every client contact, at a minimum weekly and more frequently if directed and or needed
- Follow all directives given by Randy Sams' Executive Director as well as Doorways program policies from the state and city.
- Follow the policies of the Randy Sams as stated in the employee manual and Doorways manual.
- Attend training as needed.
- Attend meetings for Doorways and Randy Sams’ as needed
- Work Closely with Randy Sams’ Residential Case Manager and the staff at the City of Texarkana, Texas
- Administer HUD funded program and assist clients in obtaining housing following grant guidelines and program policies and procedures.
- Increase clients’ awareness of available community and on-site services applicable to individual’s needs, and assist client in accessing those resources in a way that encourages personal responsibility and self-sufficiency.
- Determine together with the client, a plan of action that will be used in their individual service plan consisting of short-term objectives, long-term goals, analysis of problematic areas, suggestions for improvements and follow-up care.
- Develop client mental health or substance abuse treatment plan if indicated and document progress of each client towards plan goals in a confidential manner.
- Ensure that each client signs a copy of his or her individualized service plan.
- Provide a copy of the signed service plan to the client.
- Help the client in addressing needs, obstacles, goals, and more as it relates to their mental and physical health, employment, transportation, housing, and socialization in order to increase their chances of long-term success in maintaining their housing and stability.
- Coordinate with other service providers in the area that offer mainstream resources and ensure that applicants are enrolled in all applicable resources. Plan and implement a coordinated approach to applicants to determine the best use of funding.
- Work with staff to ensure accurate referrals and wrap-around servicing.
- Ensure confidentiality of records pertaining to any individual provided with assistance and that the address or location of any assisted housing unit is not made public.
- Function as a contributing member of a multidisciplinary team with the organization’s employees, deployed staff, volunteers and community agencies.
- Participate in monthly or weekly staff meetings and trainings as directed by supervisor, the City of Texarkana, Texas who is the lead applicant of the CoC Doorways grant, and Texas Homeless Network who the grant is provided by. (Randy Sams is a subrecipient of the lead applicant.)
- Effectively and efficiently manage complex issues that involve mental health and socioeconomic issues.
- Ensure compliance with all agency and program polices, practice effective time management and communication skills, demonstrate positive role modeling for clients and contribute to the health, well being and comfort of clients.
- Assist in budgeting the annual program finances and adhering to the budget and performance measures including monthly projections based on each clients expenses.
- Pick up, drive around, and drop off clients when needed for life skills classes or other appointments as needed, if the client cannot reasonably access transportation to the classes or appointment locations. There is a vehicle used by the Doorways program for this purpose.
- Build and maintain constructive working relationships with area housing companies and landlords in order to provide housing placement options for the Doorways clients.
- Maintain advocacy-oriented communication with the landlords and the Doorways clients in order to ensure any client needs are being addressed that are relevant to anything you may be able to help with, and that housing living conditions continue to be suitable for the clients to the best of your knowledge.
- Other duties as assigned.
Qualifications:
- Skills: Experience with providing services to the homeless and/or low-income population preferred. Bachelor’s degree preferred but will substitute experience for the right applicant. The candidate must be able to develop strong working relationships with client population, be able to relate to individuals making significant life decisions, and be supportive of individuals’ increasing independence. Must also be able to prepare written reports and maintain client documentation. Basic keyboarding and computer literacy required. Knowledge of contract management principles and practices. Ability to budget and follow a budget of expenditure performance measures and client services metrics.
- Physical Requirements: Annual TB tests required. Must also perform the majority of the work on site. Will be provided an office. Must be able to safely operate the Doorways vehicle to transport clients when needed, and have a current and valid driver’s license. Must also have a good driving record, and therefore subject to scrutiny by hiring personnel.
- Environmental Conditions: A normal office environment with little exposure to excessive noise, dust, fumes, vibrations, and temperature changes; may require some local travel; computer use; work at a fast pace with unscheduled interruptions; responsibilities require a flexible work schedule. Will have to visit client apartment homes to conduct case management, and visit with clients at other locations such as Randy Sams and other local shelters. Will be working with an at risk client population that can be at times volatile, unpredictable, and characteristic in other ways of homeless and at risk populations (drug/alcohol addictions, criminal backgrounds, health issues, etc.). May be inadvertently exposed to Covid if status is unknown at any point, TB, or other health issues, so masks will be made available for your use when it is deemed necessary or desired.
