Job Summary

Duties
- Perform thorough patient assessments including vital signs, physical examinations, and detailed medical histories to inform diagnosis and treatment strategies.
- Monitor patient conditions continuously using telemetry systems and respond swiftly to critical changes in vital signs or symptoms.
- Document all patient interactions comprehensively within Electronic Health Record (EHR) systems ensuring compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams including physicians, therapists, and case managers to develop and implement individualized care plans.
- Provide patient education on health management, medication administration, post-discharge instructions, and chronic disease management including geriatrics and memory care.
Skills
- Proven experience working with diverse patient populations with different cultural background
- Strong proficiency in EMR/EHR systems
- Ability to handle high-pressure situations confidently within emergency medicine environments
- Experience working with individuals with disabilities or developmental delays; adept at triage and acute pain management techniques.
- Excellent communication skills for patient education, documentation accuracy, and interdisciplinary collaboration.
