POSITION OVERVIEW
ATTUF Group is seeking a dedicated, reliable, and compassionate Licensed Practical Nurse (LPN) to provide targeted clinical support and medication administration for individuals with intellectual and developmental disabilities.
This role requires traveling between our two specialized group home locations in South Chicago, servicing the 60628 and 60633 zip codes. This highly flexible, low-hour role is perfect for an autonomous nurse looking for supplemental income or a predictable, part-time schedule. You will play a vital role in ensuring our residents maintain optimal health, manage chronic conditions like diabetes, and receive excellent, person-centered care.
CORE RESPONSIBILITIES
- Medication Administration: Safely administer scheduled medications to residents, with a heavy focus on subcutaneous injections, specifically insulin management and sliding-scale tracking.
- Multi-Site Coverage: Provide consistent clinical coverage across our two designated Chicago program sites (60628 and 60633 zip codes), coordinating travel to fit your rotating shift schedule.
- Documentation & Compliance: Maintain meticulous, accurate records of all medications administered. Review, update, and sign off on Medication Administration Record (MAR) sheets in strict compliance with agency policies and Illinois state regulatory frameworks.
- Routine Health Assessments: Perform basic health reviews with clients during your shifts, including checking and logging vital signs (blood pressure, pulse, temperature, blood glucose levels).
- Clinical Monitoring: Monitor residents for any changes in health status, side effects from medications, or behavioral shifts, and promptly escalate any concerns to management or the designated supervisor.
- Resident Advocacy: Provide gentle, supportive care that respects the unique communication styles, dignity, and independence of individuals with developmental disabilities.
POSITION MECHANICS & SCHEDULE
- Weekly Hours: Generates between 5 to 10 hours per week total.
- Shift Frequency: Expect to cover 2 to 3 shifts per week distributed across both working locations.
- Shift Duration: Efficient and targeted care windows lasting between 1 to 2 hours per shift.
Rotation Type: Shifts operate on a rotating basis; flexibility within your targeted windows is required to ensure continuous medical coverage for our residents.
REQUIREMENTS & QUALIFICATIONS
- Licensure: Active, unrestricted license as a Licensed Practical Nurse (LPN) in the State of Illinois is strictly mandatory.
- Location Flexibility: Must be willing and able to reliably commute to group homes in both the 60628 and 60633 zip codes.
- Clinical Skills: Strong, practical proficiency in blood glucose monitoring, sliding-scale analysis, and sub-Q insulin administration.
- Experience: Previous experience working with individuals who have intellectual or developmental disabilities is highly preferred, but not required.
- Professional Attributes: Exceptional organizational habits, accurate charting skills, precise communication, and a reliable, independent work ethic.
- Contractual Status: This is a 1099 Independent Contractor position. The successful candidate must understand that this role does not offer corporate employee benefits (such as health insurance, paid time off, or retirement matching).
