Performs occupational therapy evaluation and procedures in the home assisting patient in adapting to restricted or altered activities of daily living.
Provide physician directed occupational therapy services by initially and periodically evaluating patient’s needs, establishing a goal directed plan of care and initiating planned care. The plan of care and re-evaluation occurs no less often than every 60 days.
Provides quality care within guidelines of department policy and procedures as well as local, state and federal rules and regulations and Code of Ethics as established by the American Occupational Therapy Association.
Provides written documentation of assessment and care delivery in a professional manner according to policy and within designated time frame.
Is available during scheduled visit hours and on-call for problem resolutions for assigned patients.
Conducts supervisory visits of home health aides per department policy.
Conducts and documents case conference as needed.
Communicates patient care concerns with attending physician and the Director for Patient Services in a timely and professional manner.
Communicates with Registered Nurse case manager and other appropriate individuals involved in care to assure continuity of care.
Effectively communicates visit schedule and availability with the Director for Patient Services.
Effectively communicates with patients, their families, caregivers, etc. to ensure comprehension of care and communicates in such a manner to maintain the respect and dignity of others.
Implements a care plan that is in the best interest of the individual patient and modifies the plan as necessary for the changing needs of the patient. These changes are made according to department policies and procedures.
Enhances professional development by participating in seminars, workshops and meetings in accordance with the continuing education requirements established by state licensure laws of New York State.
Assists in training and continuing education of agency personnel as requested by supervisor.
Actively participates in agency’s performance improvement program. Actively contributes to the attainment of the agency’s mission and goals. Complies with agency policy & procedures.
Attends and participates on the agency’s Advisory Committee as requested and agreed upon with the Director for Patient Services.
Performs other job related duties as assigned.
