Position Type: Full-Time | Non-Exempt
Salary Range: $23.19 - $24.60 per hour

Position Overview:
Kings View is seeking a Care Manager (Peer or Paraprofessional) to support Managed Care services. This role coordinates care, supports members, and promotes whole-person wellness.
Key Responsibilities:
- Outreach, engagement, assessments, and care planning
- Coordinate ECM and support services
- Connect members to housing, transportation, and benefits
- Advocate with providers and agencies
- Coordinate discharge planning and referrals
- Maintain timely documentation
- Collaborate with multidisciplinary teams
What You'll Bring:
- High School Diploma or equivalent
- One year relevant experience
- Valid California Driver License
- Experience with homeless, SMI, SUD, or justice-involved populations
- BA/BS preferred; bilingual preferred
