A skilled nursing facility in New York is seeking an experienced MDS Coordinator to oversee the MDS process and support accurate clinical documentation and reimbursement.
This role is responsible for coordinating MDS assessments, ensuring regulatory compliance, and working closely with the interdisciplinary team to maintain accurate care plans and documentation. The ideal candidate is detail-oriented and experienced with the reimbursement and regulatory requirements within the skilled nursing environment.
Key Responsibilities
- Coordinate and complete MDS assessments in accordance with federal and state regulations
- Ensure accuracy and timeliness of MDS submissions
- Participate in care plan development and interdisciplinary meetings
- Monitor clinical documentation to support reimbursement and compliance
- Maintain survey readiness related to MDS and documentation practices
- Collaborate with nursing leadership and the interdisciplinary team
- Track and support quality measures related to resident care
Qualifications
- Active RN or LPN license in New York preferred
- Minimum of 3+ years of MDS experience required
- Prior Skilled Nursing Facility experience required
- Strong knowledge of Medicare, Medicaid, and reimbursement processes
- Experience with care planning and regulatory compliance
- Strong organizational and communication skills
