Under the supervision of the Practice Lead, the Nurse Client Advocate (NCA) devotes his/her best efforts to growing, building, and maintaining the services of PyxisCare Management. The NCA coordinates all aspects of a client’s needs (medical, environmental, psycho-social, financial, and legal) with the intention of improving overall health and wellbeing and to achieve the optimal quality of life for the client.
The Role of an NCA is typically a trained and licensed Nurse who “Guides the Way” for PyxisCare clients in their pursuit of exceptional health and healthcare. This role encompasses not only clinical expertise, but also through continuing client relationships, communication, availability, information, and resource management with the client.
PyxisCare does not provide traditional nursing services, but takes a consultative, multi-disciplinary approach to our clients. As our services are needed by those in urban and rural settings, day travel is expected. We also work with many different types of clients, varying ages, conditions, and socioeconomic status. A successful candidate will be able to effectively navigate and communicate with all types of people.

Essential Duties and Responsibilities
The following are fundamental duties that an individual who holds the position must perform. Other duties, in addition these essential job functions, will be assigned in the discretion of the Principal(s).
- Assessments- Conduct a multi-disciplinary health and healthcare (both in the field and virtual) assessment of the assigned client incorporating all relevant aspects as defined by PyxisCare Management, utilizing appropriate forms, screens, and protocols.
- Plan-Design an individualized plan of care with the Practice Lead to address all identified needs of the client based upon the outcome of the assessment with a full understanding of the client’s current medical status.
- Communicate-Write a detailed Assessment Report, including a summary of findings and recommendations from the field assessment within 10 days of the assessment interview.
- Implement- Attend all provider appointments, meetings, and/or conferences as dictated by the needs of the case and the parameters set forth by the PyxisCare Management policies and protocols.
- Document- Record results, activities and outcomes-Maintain accurate and complete billable hours, expense records and file records for each assigned case through our EHR. Provide assigned case updates, summaries and/or strategic plans when requested. Maintain active case notes in the EHR as directed to support all actions and expenditures. All documentation must be accurate, complete, and submitted on time.
- Other duties as assigned.

Education and Experience
- Some college: BSN, MSN preferred
- 2 + years of clinical nursing experience; Minimum of one year in one of these specialties:
- Geriatrics.
- Public Health.
- Psychiatrics.
- Neurology.
- Trauma Care.
- Supervisory or management experience.
- Utilization review and Case Management experience.
- Experience in dual - eligible benefits (Medicare and Medicaid)
Required Licensure
- Current clinical license and/or certification(s).
- Valid Driver’s License.
