Licensed Practical / Licensed Vocational Nurse (LPN / LVN)
Legacy Personnel, Inc. is currently seeking a compassionate, dependable, and skilled Licensed Vocational Nurse (LVN) to provide quality patient care in a home health setting throughout the Rocklin/Sacramento, California area.
Pay & Schedule:
$60 per visit + travel reimbursement
Part-Time - 2 visits per week
Schedule Options:
- Option 1: Monday & Wednesday
- Option 2: Sunday & Wednesday
Service Area: Rocklin/Sacramento, California
Responsibilities
- Provide skilled nursing care to patients in their homes according to established care plans.
- Perform wound care and monitor wounds for signs of infection or changes in condition.
- Assess and monitor patients' health status and report changes appropriately.
- Administer medications and treatments as ordered and within the LVN scope of practice.
- Educate patients and caregivers on appropriate care, safety, medications, and disease management.
- Maintain accurate, timely, and compliant clinical documentation.
- Communicate patient status, concerns, and progress with the supervising nurse and interdisciplinary care team.
- Follow all infection control, safety, and regulatory requirements.
- Provide compassionate, professional, and patient-centered care in the home environment.
Candidate Requirements
- Active California LVN License in good standing.
- Minimum of 1 year of LVN experience.
- Wound care experience required.
- Home health experience preferred, but not required.
- Current BLS/CPR certification.
- Valid driver's license.
- Valid auto insurance.
- Current vehicle registration.
- Ability to travel throughout the Rocklin/Sacramento service area.
- Ability to work independently while providing safe, compassionate patient care.
