Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories, to develop tailored care plans.
- Administer medications, including injections and IV infusions, following strict aseptic techniques and safety protocols.
- Provide specialized care such as wound suturing, ostomy care, catheterization, and ventilator management to support diverse patient needs.
- Educate patients and their families on disease management, medication administration, nutrition, and self-care techniques to promote independence.
- Coordinate with interdisciplinary teams for discharge planning, case management, and collaboration on complex cases involving acute pain management or infectious disease care.
- Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner while adhering to HIPAA regulations.
- Manage emergency situations confidently through triage skills and critical care experience, including responding to acute episodes in home settings.
Requirements
- Valid RN license with current state registration and compliance with licensing standards.
- Proven experience in home health nursing or related fields such as hospital medicine, ICU, or emergency medicine.
- Strong clinical skills including IV insertion, phlebotomy, laboratory specimen collection/processing, and sterile processing techniques.
- Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation.
- Knowledge of complex medical procedures such as dialysis support, cardiac catheterization post-care, or trauma medicine is highly desirable.
- Ability to perform physical assessments and vital sign monitoring accurately while demonstrating anatomy knowledge and physiology understanding.
- Excellent communication skills for patient education and working collaboratively with multidisciplinary teams; experience working with individuals with disabilities or in geriatrics is a plus.
