Duties
- Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop tailored treatment plans.
- Implement therapeutic interventions such as manual therapy, trigger point therapy, and patient education to facilitate recovery and improve functional outcomes.
- Coordinate discharge planning and collaborate with physicians, nurses, and other healthcare providers to ensure seamless continuity of care across settings.
- Manage clinical documentation standards by accurately recording patient progress, treatment details, and home care instructions using electronic health records (EHR) systems.
- Conduct patient care activities in various environments including home settings, outpatient clinics, inpatient hospitals, and outpatient rehabilitation centers.
- Assist patients with mobility aids and orthopedics-related therapies while addressing specific needs related to geriatrics, pediatrics, neurology, or orthopedics.
- Engage in ongoing professional development by staying current with outpatient experience trends, inpatient hospital protocols, and community care practices.
Qualifications
- Valid California license or certification as a Physical Therapist with practice in a physical therapy role.
- Proven experience in home health care or outpatient rehabilitation settings with familiarity in outpatient clinic experience.
- Strong understanding of medical terminology, physiology, anatomy, and clinical documentation standards.
- Experience working with geriatric patients and knowledge of geriatrics-specific treatment approaches.
- Demonstrated ability to develop treatment plans for diverse conditions such as neurology or orthopedics while adhering to healthcare regulations.
- Excellent client communication skills to educate patients on their care plans and promote active participation in recovery.
- Proficiency in EMR (Electronic Medical Records) systems or other electronic health records (EHR) management tools.
