Responsibilities:
- Provide comprehensive home health nursing care to patients in their homes.
- Monitor patient progress and adjust care plans accordingly.
- Document patient interactions accurately using OASIS and Homecare Homebase EMR systems.
- Educate patients and families on health management and disease prevention.
- Collaborate with interdisciplinary teams to ensure quality patient outcomes.
