Overview
Join our compassionate healthcare team as a Certified Home Health Aide (CHHA) and make a meaningful difference in the lives of clients requiring personalized care in their homes. Operating under the direction and supervision of a Registered Nurse (RN), this vital role focuses on providing essential support with Activities of Daily Living (ADLs) to individuals with diverse needs, promoting independence, comfort, and dignity. As a CHHA, you will rely on strong communication skills to build meaningful connections with clients and families while maintaining close collaboration with the supervising RN and care team to deliver high-quality, patient-centered care.
Key Responsibilities
- ADL Assistance: Assist clients with core Activities of Daily Living (ADLs), including bathing, dressing, grooming, personal hygiene, toileting, and mobility/transfer support utilizing proper transfer techniques and equipment (e.g., Hoyer lifts).
- Proactive Communication & Observation: Maintain continuous, clear, and compassionate communication with clients and family members. Observe and promptly report any physical, emotional, or behavioral changes in client condition to the supervising RN.
- Cognitive & Memory Support: Provide empathetic, specialized care for clients with dementia, Alzheimer's disease, or cognitive impairments using supportive communication and memory care techniques.
- Household Support: Perform light housekeeping tasks tailored to client needs, including light cleaning, laundry, meal preparation, and maintaining a clean, safe, and comfortable home environment.
- Accurate Documentation: Utilize Electronic Health Record (EHR) and Electronic Visit Verification (EVV) systems to accurately and timely document care activities, adherence to the plan of care, and daily notes.
- Mobility & Independence: Safely assist clients with ambulation and community/home support to encourage maximum independence and quality of life.
Qualifications & Skills
- Required Certification: Current, active New Jersey Certified Home Health Aide (CHHA) certification in good standing.
- Communication Expertise: Exceptional verbal and written communication skills with a empathetic, patient-centered approach; strong active listening skills and strict adherence to HIPAA and client confidentiality standards.
- ADL & Transfer Proficiency: Demonstrated experience assisting with personal care, hygiene, and safe client transfer methods.
- Team Collaboration: Proven ability to work independently in home environments while following an RN-directed plan of care and maintaining effective ongoing communication with the clinical supervision team.
- Specialized Care Knowledge: Familiarity with dementia care techniques, senior care best practices, and memory support strategies.
- Technical Familiarity: Comfort using EMR/EHR/EVV mobile apps and digital documentation tools for home care reporting.
