Responsibilities
- Conduct comprehensive patient evaluations to determine functional limitations and develop tailored care plans
- Implement evidence-based physiotherapy techniques such as trigger point therapy, manual therapy, and therapeutic exercises
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient care transitions
- Maintain accurate clinical documentation in accordance with healthcare standards
- Educate patients and their families on injury prevention, mobility strategies, and home care routines
- Manage outpatient clinic sessions, inpatient hospital stays, or home health visits depending on the setting
- Monitor patient progress regularly and adjust treatment plans to optimize recovery outcomes
Experience
- Proven practice in a physical therapy role with strong knowledge of physiology, anatomy, and medical terminology
- Prior experience working with geriatric patients in outpatient rehabilitation centers or home & community care settings
- Hands-on experience with acute care environments such as hospitals or inpatient units
- Familiarity with outpatient clinic operations and outpatient rehabilitation services
- Knowledge of clinical documentation standards
- Experience in specialized areas such as orthopedics, neurology, or trigger point therapy is highly desirable
- Demonstrated ability to work effectively within multidisciplinary teams to deliver exceptional patient care
