Position Summary
Greystar Healthcare Services LLC is seeking a compassionate, skilled, and experienced Registered Nurse (RN) to join our growing Home Health team. The ideal candidate will possess extensive experience in wound care management, negative pressure wound therapy (Wound VAC), patient assessment, care planning, and chronic disease management.
Essential Duties and Responsibilites
- Perform comprehensive nursing assessments and develop individualized plans of care.
- Provide skilled nursing services in accordance with physician orders and agency policies.
- Assess, treat, and monitor acute and chronic wounds.
- Initiate, manage, and troubleshoot Wound VAC systems.
- Perform wound measurements, staging, documentation, and assessments.
- Administer medications, injections, IV therapy, catheter care, and skilled nursing procedures.
- Educate patients and caregivers regarding wound care and disease management.
- Coordinate care with physicians and interdisciplinary team members.
- Complete OASIS assessments and maintain timely documentation.
- Ensure compliance with Medicare, Medicaid, and agency requirements.
Qualifications
Current and unrestricted Texas RN License.
- Minimum two (2) years nursing experience.
- Home Health experience preferred.
- Advanced wound care and Wound VAC experience required.
- Current BLS Certification.
- Valid driver's license and reliable transportation.
Preferred Qualifications
- Wound Care Certification (WCC, WOCN, CWCN, or equivalent). OASIS documentation experience.
- PICC Line, IV Therapy, and Infusion Therapy experience.
- Knowledge of Medicare and Medicaid regulations

Pay: $25.00 - $35.00 per hour
