CC Young, a vibrant senior living community and a non-profit leader in senior living and Health Care, is seeking a Hospice Chaplain to provide compassionate spiritual, emotional, and pastoral care to hospice patients, families, caregivers, and hospice staff regardless of religious affiliation, cultural background, or personal beliefs. The Chaplain is an active member of the Interdisciplinary Group (IDG) and assists patients and families in finding meaning, hope, comfort, and peace throughout the end-of-life journey.
Job duties include, but are not limited to:
- Complete comprehensive spiritual assessments upon admission.
- Develop and implement individualized spiritual care plans based on patient goals and preferences.
- Provide routine spiritual support through home visits, facility visits, telephone contacts, and virtual visits when appropriate.
- Support patients and families experiencing grief, anticipatory grief, anxiety, fear, anger, forgiveness issues, reconciliation, and life review.
- Facilitate prayer, scripture reading, sacraments, rituals, memorial services, and other faith practices as requested.
- Respect and support individuals of all faith traditions or those with no religious affiliation.
- Assist patients in identifying personal sources of hope, meaning, dignity, and peace.
- Respond promptly to spiritual crises and end-of-life situations.
- Participate in all required IDG meetings.
- Communicate spiritual assessment findings and recommendations to the care team.
- Collaborate with physicians, nurses, social workers, aides, volunteers, and bereavement staff.
- Assist with development and revision of the patient's Plan of Care.
- Identify patients requiring increased spiritual support.
- Coordinate care with community clergy when requested by the patient or family.
- Develop and maintain relationships with churches, synagogues, mosques, temples, and other faith communities.
- Complete timely documentation in the electronic medical record.
- Document spiritual assessments, interventions, patient responses, goals, and outcomes.
- Maintain compliance with Medicare Conditions of Participation and agency policies.
- Ensure documentation supports the Plan of Care.
- Identify families at risk for complicated grief.
- Participate in memorial services and remembrance events.
- Provide spiritual support during and immediately following a patient's death when requested.
- Provide emotional and spiritual support to hospice staff.
- Participate in employee memorials, debriefings, and wellness initiatives.
- Assist with education related to compassion fatigue, resilience, grief, and self-care.
- Educate patients, families, staff, volunteers, and community groups regarding hospice philosophy and spiritual care.
- Participate in orientation and ongoing staff education.
- Perfom other duties as assigned.
Minimum qualifications include:
- Master's Degree in Divinity (M.Div.), Theology, Pastoral Care, or related field preferred.
- Clinical Pastoral Education (CPE) preferred.
- Minimum two (2) years of pastoral ministry, chaplaincy, or hospice experience.
- Hospice or palliative care experience preferred.
- Ordained, commissioned, endorsed, or otherwise recognized by an established faith tradition.
- Board Certification (APC, NACC, NAJC, or equivalent) preferred but not required.
- Valid driver's license, reliable transportation, and automobile insurance.
- Ability to minister to individuals of all cultures and belief systems.
- Strong crisis intervention and grief support skills.
- Understanding of hospice philosophy and end-of-life care.
- Ability to work independently while functioning as an effective interdisciplinary team member.
- Strong organizational and documentation skills.
- Proficiency with electronic medical records.
Physical Requirements
- Frequent driving throughout the service area.
- Ability to lift up to 25 pounds.
- Ability to sit, stand, walk, bend, kneel, and climb stairs.
- Ability to enter patient homes, nursing facilities, hospitals, and assisted living communities.
