As an RN, you'll work alongside an interdisciplinary team to provide high-quality nursing care, support direct care staff, and advocate for the health and well-being of the individuals we serve.
What You'll Do
- Conduct initial and ongoing nursing assessments.
- Develop and implement individualized care plans.
- Administer medications and treatments as prescribed by physicians or Advanced Practice Providers (APPs).
- Monitor individuals' health status and respond appropriately to changes in condition.
- Provide clinical oversight and guidance to LPN/LVNs and direct support staff.
- Delegate nursing tasks appropriately while ensuring competency and compliance.
- Maintain medication inventory, storage, preparation, and documentation.
- Coordinate care with physicians, pharmacies, specialists, and other healthcare providers.
- Participate in interdisciplinary team meetings, care reviews, and service planning.
- Provide health education and training to staff, caregivers, and individuals served.
- Complete accurate, timely, and confidential clinical documentation in accordance with HIPAA and company policies.
- Support regulatory compliance and quality improvement initiatives.
- Participate in an on-call rotation, providing telephone triage support after hours as assigned.
- Respond promptly to medical emergencies and ensure a safe environment for individuals and staff.
Qualifications Required
- Associate Degree in Nursing (ASN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program.
- Active Registered Nurse (RN) license in your state of residence.
- If working in another state, eligibility for a multistate RN license through the Nurse Licensure Compact (NLC).
- Minimum of one (1) year of clinical nursing experience.
- Current Basic Life Support (BLS) certification.
- Valid driver's license and proof of automobile insurance.
Preferred
- Experience working with individuals with Intellectual and Developmental Disabilities (IDD/DD).
- Knowledge of CMS, HCBS Waiver, ICF/IID, and other state and federal IDD regulations.

Work Environment
This position provides nursing services in residential homes, day programs, community settings, and administrative offices. Travel between service locations is required.
Physical requirements include:
- Frequent standing and walking.
- Ability to lift or assist with up to 50 pounds.
- Bending, reaching, twisting, and assisting individuals with activities of daily living.
- Daily computer and electronic health record documentation.
- Ability to respond quickly to emergencies.
- Potential exposure to bloodborne pathogens and communicable diseases. Appropriate PPE is provided.
