Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs, mental status, and overall condition; report changes promptly
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Support medication administration following care plans while adhering to HIPAA regulations for patient privacy
- Operate assistive devices such as Hoyer lifts and ensure safe transfer techniques for residents with mobility challenges
- Maintain a clean and safe environment by performing light housekeeping, laundry, and sanitation tasks
- Document patient observations, care provided, and any incidents accurately in compliance with healthcare standards
Skills
- Proven experience in home health, assisted living, nursing homes, or group homes; familiarity with long-term care environments preferred
- Strong knowledge of patient observation, vital signs monitoring, medication administration, and dementia or Alzheimer's care
- Ability to work effectively with individuals with developmental disabilities, autism, or other special needs; experience in hospice or memory care is a plus
- Excellent communication skills for collaborating with healthcare teams, residents’ families, and social workers
- Physical stamina for heavy lifting and assisting residents with mobility; comfortable using equipment like Hoyer lifts
- Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information
- CPR and First Aid certification required; additional certifications such as working with disabilities or specialized patient care are advantageous
