What You'll Be DoingDeliver Exceptional Therapy
- Complete in-home assessments focused on mobility, balance, and strength
- Develop and implement individualized treatment plans
- Provide therapeutic interventions and exercise progression
- Recommend adaptive equipment and safety modifications
- Educate patients and caregivers to maximize outcomes
Collaborate Across The Continuum
- Partner with nurses, aides, physicians, and interdisciplinary teams
- Communicate progress and adjust treatment plans as needed
- Document visits and monitor patient outcomes
- Help patients remain strong, safe, and independent
Required Qualifications
- Active Minnesota Physical Therapist license
- Valid driver's license and reliable transportation
- Strong communication and clinical reasoning skills
- Ability to work independently and manage your schedule effectively
Preferred Qualifications
- Home health experience
- Outpatient, inpatient, SNF, or rehabilitation experience
- Passion for relationship-based, patient-centered care
