About the Role
As a Part-Time or PRN Medical Social Worker, you will provide skilled social work services to patients in their homes throughout Miami in accordance with physician orders, Medicare guidelines, and agency policies. You’ll work independently in the field while being fully supported by our Clinical Managers, QA team, and administrative staff.
Key Responsibilities
- Conduct psychosocial assessments and develop individualized care plans
- Assess patient and family needs related to social, emotional, financial, and environmental factors
- Provide counseling, crisis intervention, and emotional support to patients and caregivers
- Assist with community resource coordination, including long-term planning and discharge needs
- Educate patients and families on advance directives, community services, and available support resources
- Accurately complete all documentation in the EMR within required timeframes
- Collaborate with physicians, nurses, therapists, and interdisciplinary team members
- Participate in case conferences and care coordination as needed
- Maintain compliance with Medicare, ACHC, and agency standards
- Current FL licensure
- Minimum 1 year of clinical experience; home health or healthcare setting preferred.
- Working knowledge of community resources, entitlement programs, and care coordination practices.
- Proficiency with electronic medical records preferred. (Kinnser Wellsky)
- Strong communication, problem-solving, and interpersonal skills.
- Reliable transportation, valid driver’s license, and auto insurance.
- Ability to work independently and manage a flexible field-based schedule.
