Position Summary:
The Hospice RN Case Manager provides compassionate, patient-centered care to individuals and families facing end-of-life needs. This role coordinates and delivers skilled nursing care and supportive services in accordance with the patient’s plan of care, ensuring comfort, dignity, and quality of life.
Key Responsibilities:
To plan, coordinate and provide hospice care in accordance with the patient's plan of care.
Clinical Accountabilities
- Ensure the delivery of high-quality, compliant hospice care.
- Review patient care plans and documentation for accuracy and appropriateness.
- Coordinate with interdisciplinary team members, including physicians, social workers, and chaplains.
- Coordinates hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
- Consult with the attending physician and Hospice Medical Director concerning changes in patients’ condition requiring order changes and/or changes in the plan of care.
- Participate in IDG and case conferences
- Discuss with the supervisor if IDG determines that the patient would benefit from contracted services.
- Actively participates in QAPI meetings and activities.
- Submit complete, accurate and relevant clinical notes in a timely manner.
- Take responsibility for ensuring patients’ plans of care are kept current.
Regulatory Compliance
- Maintain knowledge of and adherence to local, state, and federal hospice regulations.
- Participate in audits, surveys, and quality improvement initiatives.
- Ensure timely and accurate documentation consistent with Medicare and accreditation standards.

Pay: $70,000.00 - $90,000.00 per year
