We are seeking a compassionate and highly skilled Assisted Living Registered Nurse (RN) to join our dedicated healthcare team. Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to develop personalized care plans.
- Monitor residents for acute pain management needs, infectious disease symptoms, and other health concerns, escalating issues promptly.
- Coordinate with interdisciplinary teams for discharge planning, case management, and long-term care strategies aligned with NCQA standards.
- Maintain detailed medical documentation using EMR/EHR systems like ECP while ensuring compliance with HIPAA regulations.
- Support emergency response efforts by managing airway management, ventilator use, or trauma medicine protocols in urgent situations.
- Educate residents and their families on health coaching topics such as medication management, nutrition, and chronic disease management.
- Ensure infection control practices are rigorously followed to protect residents and staff alike.
Experience
- Proven nursing experience in assisted living facilities or long-term care settings is preferred.
- Familiarity with EMR/EHR systems like ECP is essential for accurate medical documentation.
- Experience working with individuals with Elderly, Physically Disabled, Dementia
- Prior roles involving post-acute care or hospice & palliative medicine will strengthen your application.
Pay: $33.00 - $37.00 per hour
