Job Summary:
As a Clinical Specialty Pharmacist, you will play a critical role in ensuring the safe and effective use of specialty medications and supporting patients and/caregivers to feel empowered and confident in their treatments. You will participate in the clinical and operational pharmacy workflow, managing required prescription review steps, providing clinical oversight, performing clinical consultations and assessments, and serving as a resource to both non-clinical staff and external healthcare providers. This role combines traditional pharmacist responsibilities with active collaboration across interdisciplinary teams to support optimal patient outcomes.
Candidates are required to live in a commutable distance from the pharmacy. This position is required to be available to work onsite Monday - Friday between the hours of 8:30am and 5:00 pm MT. This position includes providing on-call remote after hours support for patients and prescribers.
Required Qualifications:
- Active Pharmacist license in good standing issued by the Arizona State Board of Pharmacy
- Doctor of Pharmacy (Pharm.D.)
- Minimum 1 years of professional experience as a pharmacist
- Strong knowledge of state and federal pharmacy regulations and compliance standards
- Excellent communication skills for engaging with patients, providers, and team members
Preferred Qualifications & Competencies:
- Active Pharmacist license in good standing issued by the New York State Board of Pharmacy
- Experience in specialty pharmacy or clinical care setting, especially with case management and/or polypharmacy care models
- 3+ years of experience as a pharmacist, with experience managing pharmacy operations
- Minimum 1 year of professional experience as a specialty pharmacist, in an accredited specialty pharmacy
- Familiarity with URAC and ACHC specialty accreditation standards
- Outstanding problem-solving and interpersonal skills
Key Responsibilities:
- Act as a compassionate, trusted member of the patient’s care team by building meaningful connections with patients and caregivers, listening to their concerns, and supporting them throughout their treatment journey.
- Conduct initial and ongoing clinical assessments using validated tools to identify potential clinical risks, barriers to adherence, or care needs. Use these assessments to inform personalized care planning.
- Deliver comprehensive medication counseling that empowers patients and caregivers to understand their treatment regimen, side effects, and goals of therapy—supporting shared decision-making and long-term adherence.
- Monitor and evaluate patient response to therapy, including documentation of progress, adverse events, adherence challenges, and opportunities for therapeutic optimization.
- Collaborate with prescribers, nurses, care coordinators, and payers to support medication access, treatment optimization, and timely resolution of access-related issues.
- Support the development and execution of individualized care plans in accordance with URAC and ACHC Clinical Management Program standards, ensuring care is coordinated, clinically appropriate, and aligned with patient goals.
- Participate in the identification, documentation, and mitigation of medication-related problems, ensuring safe and effective use of specialty therapies.
- Engage in continuous quality improvement activities by collecting and reviewing patient-reported outcomes and contributing to performance improvement plans.
- Serve as a clinical leader on shift, offering guidance to pharmacy and non-clinical staff, resolving clinical escalations, and ensuring operational excellence.
- Ensure compliance with all relevant federal and state regulations, accreditation requirements (URAC, ACHC), and internal protocols governing specialty pharmacy operations.

Pay: $135,000.00 - $145,000.00 per year
