Position Summary
We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our home health team. This position involves providing skilled nursing visits to clients in their homes. Visits may include services such as medication administration, insulin injections, catheter care, diabetes management, wound care, patient assessments, and family education.
Nurses are assigned based on their preferred service area and patient needs. This is a great opportunity for an LPN who is interested in starting part-time with the potential to transition into a fuller schedule as patient assignments become available.
Service Area
Nurses may be assigned to patients in their designated service area based on availability, location, and patient needs.
Service areas may include:
- East Valley
- West Valley
- Central Phoenix
- Tucson
- Other surrounding areas as needed
Key Responsibilities
- Provide skilled nursing care in the patient’s home according to the plan of care.
- Perform nursing services such as insulin administration, catheter care, wound care, diabetes management, medication management, and other ordered interventions.
- Complete patient assessments and report changes in condition as appropriate.
- Educate patients and families regarding medications, treatment plans, disease management, and safety precautions.
- Follow all agency policies, patient-specific instructions, and physician orders.
- Complete accurate and timely documentation for each visit.
- Comply with Electronic Visit Verification requirements.
- Communicate professionally with patients, families, office staff, clinical leadership, and members of the care team.
- Report incidents, patient refusals, changes in condition, missed visits, delays, or concerns according to agency policy.
- Maintain professionalism, confidentiality, and patient dignity at all times.
- Respond appropriately to urgent patient needs or emergencies.
Schedule
- Part-time and PRN opportunities available.
- Flexible/open availability is preferred to help support patient scheduling needs.
- Monday through Friday availability preferred.
- Weekend availability may be needed depending on patient assignments.
- Schedule may vary based on patient needs, nurse availability, and assigned East Valley patient schedules.
- Potential to transition into a fuller schedule as additional patient assignments become available.
Qualifications
- Current and unrestricted Licensed Practical Nurse license in the State of Arizona.
- Current CPR/BLS certification.
- Valid Arizona Fingerprint Clearance Card.
- Article 9 certification, or willingness to obtain prior to patient assignment.
- At least one year of home health, private duty, developmental disability, or related nursing experience preferred.
- Knowledge of Electronic Visit Verification preferred.
- Strong communication and documentation skills.
- Ability to work independently in a home health setting.
- Reliable transportation.
- Valid driver’s license and vehicle insurance, if traveling to patient homes.
- Ability to manage time effectively and follow scheduled visit times.
- Comfortable using technology for communication, documentation, and electronic forms, including Microsoft Teams, Kinnser/WellSky EMR, Adobe electronic forms/signatures, and other agency-required systems.
- Excellent organizational skills and attention to detail.
- Commitment to providing high-quality care to individuals with developmental disabilities and/or complex care needs.
Physical Requirements
This role may require the ability to assist with patient positioning, transfers, mobility support, and other physical tasks related to patient care. Nurses must be able to safely perform assigned duties in the home setting and request assistance when needed.
