Life Wheels On, a specialty wheelchair and mobility practice in partnership with Covell Care, is seeking a Colorado-licensed Physical Therapist or Occupational Therapist to join our growing team as an independent contractor. This is an excellent opportunity for clinicians who enjoy working one-on-one with clients, value autonomy, and want to make a meaningful difference in the lives of individuals with complex mobility needs.
What You'll Do
As part of our team, you'll provide personalized services in clients' homes and through telehealth, helping individuals maximize independence, safety, and function.
Responsibilities include:
- Performing comprehensive mobility and wheelchair evaluations
- Assessing home environments and identifying accessibility needs
- Providing equipment education, training, and follow-up services
- Collaborating with physicians, ATPs, suppliers, caregivers, and other healthcare professionals
- Completing timely, accurate clinical documentation
- Supporting clients through the mobility equipment process from evaluation to delivery when appropriate
- Providing ongoing therapy for a variety of diagnoses under the "outpatient in-the-home" model of care

Qualifications
- Current Colorado license as a Physical Therapist or Occupational Therapist
- Excellent clinical judgment and communication skills
- Ability to work independently and manage your schedule
- Comfortable providing care in home and virtual environments
- Passion for helping individuals with mobility and accessibility challenges
- Wheelchair seating or complex rehabilitation technology experience is welcomed but not required—training is available for the right candidate.
Position Information
- Employment Type: Independent Contractor
- Location: Colorado Springs and surrounding communities
- Caseload: Approximately 5–10 visits per week initially, with opportunity for growth
- Visit Format: Home visits and telehealth
- Compensation: Percentage-based reimbursement (starting around 55-60% of collected reimbursement, based on experience and productivity)
- Scheduling: Flexible; visit length determined by clinical need
