Job Title: Direct Support Professional
Department: Supportive Living
Reports to: Supportive Living Case Manager
FLSA Classification: Non-Exempt
Job Type: FT / PT / PRN
Hours: Any (12AM-11:59PM) available, (4PM-8AM) times most needed
Days: Any (Sunday-Saturday) available
Salary: $13.00 base pay which will increase to $14.75 after completion of agency paid training.
Job Summary/Objective:
Assist those who need support to lead self-directed lives and to participate fully in their community. Honor the individual path suggested by the unique gifts, preferences, and needs of each person supported. Partner with persons supported and those who are significant to them to lead a life of contribution, freedom, opportunity, and well-being. Our supportive living program runs 24/7. This allows flexible, individualized scheduling for potential employees and their current needs.
Job Description/Responsibilities:
Follow the persons supported plans (Individual Support Plans, Behavior Support Plans, Nutritional Plans, Therapy Plans, etc.) to accomplish the following:
1) Transport and accompany community outings and/or medical appointments
2) Administer medications
3) Complete documentation
4) Prepare meals (breakfast, lunch, dinner, etc.)
5) Supervise and/or assist eating
6) Assist with toileting (changing and/or transferring)
7) Assist with personal care (dressing, grooming, hygiene, etc.)
8) Goals, Outcomes & Objectives
9) Household care (laundry, bathrooms, kitchen, dishes, sweep, mop, garbage, etc.)
10) Call-Ins to the Center (12AM, 2AM, 4AM)
11) Bed checks (hourly and/or every other hour)
12) Change and/or reposition (hourly and/or every other hour)
13) Alert supervisor of needed supplies (gloves, wipes, briefs, cleaners, medications, paperwork, etc.)
14) Complete all notes & documentation daily (elimination, sleep, menus, behavior, MARS, etc.)
Essential Functions:
· Provide person-centered supports by implementing all of the persons supported plans in a collaborative and expeditious manner through a range of participatory planning techniques
· Promote physical health & wellness by providing personal care
· Practice integrity & responsibility by reporting immediately any person supported incident on the proper forms and to the proper persons as outlined in the Reportable Event Management Policy and Procedures
· Identify crises and use knowledge about crisis prevention, intervention, and resolution techniques while complying with regulations for reporting
· Ensure confidentiality by respecting the privacy of privileged information of persons supported
· Exercise equity, fairness, and justice for persons supported and the community
· Demonstrate respect by fostering pride in the homes of our persons supported by assisting with household operations, such as housekeeping and meal preparation
· Recognize importance of friends, family and community relationships while promoting community living skills to build/maintain relationships by matching specific supports and interventions to unique needs
· Assist with community & service networking by accompanying persons supported as necessary to access resources and to participate in community activities (appointments, community recreation/leisure resources, shopping, etc.)
· Empower self-determination, self-esteem and assertiveness to make own decisions
· Adhere to detailed schedule of support that reflect persons supported goals, outcomes and objectives
· Use effective advocacy strategies to overcome challenges by mobilizing resources to assist reaching goals (including but not limited to education, vocation, career support, etc.)
· Foster professional and timely communication between persons supported, families, other Direct Support Professionals, and Case Manager to establish collaborative relationships
· Prioritize organizational participation by attending mandatory agency trainings and meetings to ensure quality practices and supports
· Complete and maintain accurate and timely records for all required documentation (reports, medication administration records, daily notes, progress notes, attendance logs, etc.)
· Respond to needs, desires and interests through knowledgeable formal and informal assessment practices
· Encourage self-development by identifying areas for self-improvement, pursuing necessary educational/training resources, and sharing knowledge with others

Physical Demands:
· Ability to frequently bend, stoop, pull, push, reach, sit, twist, turn, grasp, feel, talk, hear, bear weight, drive, and lift up to 50 pounds

Required Education/Certification:
· Proof of High School Diploma, GED or equivalent
o CPR/First Aid Certificate (training available upon hiring)
o Medication Administration Certificate (training available upon hiring)
Additional Eligibility Requirements:
· Proof of Valid TN Class D Driver’s License with clean driving record
· Proof of 18 years of age or older
· Pass Background Check
· Pass Drug Screening
· Pass Tuberculosis Screening
